そのような場合、3パターンの抗ウイルス剤の使用法があります。
① 発症してから薬剤を5日間内服する方法（その都度受診し、処方を受ける。）
② 「ピリピリ、チクチク」といった前兆に気づいたら、患者の判断で服用を開始する方法（PIT）
③ 1日1回を継続投与する方法（再発抑制療法）
の3パターンがあります。（③は性器ヘルペスのみに適応）。

PIT（patient initiated therapy）は 再発の症状に気づいたら患者自身の判断で始めるヘルペス治療法で健康保険で認められています。
メリットとして、より早期にヘルペス再発への対処が可能となり、1日分の服薬で治療が完結できることです。また、ヘルペスの前兆を感じたが、忙しくて受診できない場合に、自分の判断で治療が開始できます。
再発性のヘルペスでは、前兆を感じる時点ですでにウイルスの増殖があり、2日目がウイルス量のピークとなります。ウイルス増殖期にしっかり作用させることで、症状の悪化を防ぐことが期待されます。
使用するお薬は、「ファムビル」と「アメナリーフ」の2種類あります。
ファムビルでPITする場合
・再発の初期症状(ヘルペスの前兆)を正確に判断可能な患者さんが対象。
・再発頻度が年間概ね3回以上 であること、
・腎機能の低下がないこと。
・前兆出現後6時間以内に、一回に本剤4錠を服用し、さらに 12時間毎にもう一度服用します。
・一回分の薬剤費用の自己負担（3割負担の方の例では）700円弱となります（先発品のみで認められる用法。ジェネリック医薬品では不可）。
アメナリーフでPITする場合
・再発の初期症状を正確に判断可能な患者さんが対象。
・症状出現後（皮疹がまだでていない状態）6時間以内に、本剤を1回に6錠服用します。
・空腹時の服用では吸収が悪いため、食後（あるいは間食を食べてから）にのむようにします。
・本剤は腎機能が低下していても同じ量服用できる点もメリットです。
・アメナリーフは1回で必要十分量を投与することで、ウイルス増殖期に作用します。
・一回分の薬剤費用の自己負担（3割負担の方の例では）2200円弱となります(ジェネリック医薬品がないお薬)。
なお、どちらも
・保険適応上、「1か月分に1回分」の処方のみ可能で、まとめてお渡しできませんので注意ください。

In such cases, there are three patterns of antiviral drug usage:

Regarding PIT (Patient Initiated Therapy):
PIT is a herpes treatment method recognized by health insurance, where patients themselves initiate treatment upon recognizing recurrent symptoms.
Benefits include the ability to address herpes recurrence more promptly, and the convenience of completing treatment with a single day’s medication. Additionally, for individuals who sense herpes prodromal symptoms but are too busy for a clinic visit, initiating treatment based on their judgment is possible.
In recurrent herpes, viral replication begins at the onset of prodromal symptoms, with the second day being the peak of viral load. By acting during the viral replication phase, it is anticipated that symptoms’ worsening can be prevented.
Two medications, Famvir Tablets and Amenalief Tablets, are used in this context.
For PIT with Famvir:
Targeted for patients who can accurately identify the early symptoms (prodromal signs) of recurrent herpes.
The recurrence frequency should generally be three times or more per year.
No impairment in renal function.
Take 4 tablets of the medication within 6 hours of prodromal symptoms appearing, and repeat every 12 hours.
The self-payment cost for a single dose is approximately 700 yen (applicable only for the brand-name product; not permitted for generic drugs).
For PIT with Amenalief:
Targeted for patients who can accurately identify early symptoms of recurrence.
Take 6 tablets of the medication within 6 hours of symptoms appearing (before the rash develops).
Take the medication after eating, as absorption is poor on an empty stomach.
The medication can be taken in the same amount even with impaired renal function.
The self-payment cost for a single dose is approximately 2,200 yen (no generic drugs available).
